Being discharged from hospital and needing support? Coming home after time in hospital can feel like a big adjustment, especially without family or friends nearby to lean on.
At Cosy Home Care, we help guide you to a place you can call home. We coordinate your care from the moment you're discharged, working alongside your hospital and clinical team to ensure a smooth transition, then continue supporting you at home while you recover.
Who is this program for?
We support NDIS participants who:
- Are medically ready to leave hospital but need housing or support in place first
- Have an approved NDIS plan
- Need SIL, SDA, MTA, STA, or Respite Care
- Have complex physical, neurological, intellectual, or psychosocial support needs
- Need help transitioning safely into the community
- Are at risk of delayed discharge due to housing or support barriers
What is The Process
Making a Referral
The journey starts with a referral, whether that's from a hospital, discharge planner, support coordinator, allied health professional, family member, or the participant themselves. Our team takes the time to understand each person's situation and how we can best support their transition.
Getting to Know You
Every participant has their own story, goals, and aspirations.
We take the time to understand:
- Accommodation requirements
- Support needs
- NDIS funding
- Personal preferences
- Long-term goals
Finding the Right Fit
We work collaboratively to identify accommodation and support options that align with the participant's needs, lifestyle, and goals, so they feel comfortable,supported, and at home.
